- The child is under 19 years old.
- The child is a California resident.
- The family’s total income falls within the income guidelines.
- The child has not been covered under the parent’s employer-sponsored health insurance in the last 3 months.
- The child is not eligible for or receiving no-cost Medi-Cal.
